Comagine Health has been awarded a contract with the federal Department of Health and Human Services (HHS) Administration for Community Living (ACL) to improve chronic disease self-management education (CDSME) in Nevada.
Our work will focus on establishing a hub in Nevada to support local chronic disease self-management program delivery organizations by increasing program reach, providing data management services, coordinating provider referrals and identifying additional opportunities for sustainable funding.
The ACL grant provides the funds needed to ensure programs and delivery partners are able to benefit from a coordinated network, clarify and streamline functions and roles of key partners, and improve the reach and sustainability of embedded programs in a broader set of services addressing social determinants of health.
Comagine Health will collaborate on this work with Dignity Health, Access to Healthcare Network and other community partners.
In 2021, Comagine Health received a similar ACL grant to improve CDSME in Oregon.
