Please join the Comagine Health Six Building Blocks team on Feb. 17 at 12 p.m. PST for a short session focusing on the social determinants of health and pain management. This event will feature Comagine Health Senior Improvement Advisor Maria Klemesrud, M.Ed, LMHC.
Some questions that will be addressed include:
What are the connections between social determinants of health and chronic pain?
What strategies can be used within a clinic settling to effectively address social risk factors of patients?
What is the value of connecting patients to resources?
Do implicit biases impact patient care?
You will also learn about how the Six Building Blocks Framework practice facilitators can assist your program. A quickinar is a brief 20-30 minute webinar presentation to provide a comprehensive overview of a defined topic area.
What are the Six Building Blocks?
The Six Building Blocks Opioid Management Program is a team- and evidence-based quality improvement roadmap to support practices as they implement effective guideline- and team-based care for their patients with chronic pain who are using opioid therapy.
Developed by the University of Washington of Family Medicine and the Kaiser Permanente Washington Health Research Institute, Six Building Blocks focuses on six key areas in guiding opioid management processes.
Comagine Health is carrying this work forward into 2022 through a project with the Washington State Department of Health.
