Comagine Health, in partnership with the Utah Geriatric Education Consortium, is offering training sessions regarding advance care planning (ACP) and end of life best practices for professionals working in hospice, home health and any other long-term and post-acute care (LTPAC) setting.
The training will be led by Jean Lyon, PhD, APRN, a clinical care practitioner and senior infection prevention advisor at Comagine Health. Dr. Lyon is a family nurse and psychiatric mental health nurse practitioner with a master’s degree in community health nursing and a PhD in Nursing with a focus on administration from the University of California, San Francisco. She will cover the basic principles of ACP as well as tools and resources for serious illness conversations in LTPAC settings.
This training is offered twice and includes two open office hours sessions to ask additional questions and practice conversations skills. You only need to register once.
