Webinar: Using Community Resources for Improving Care Transitions

Comagine Health invites community partners, physician offices and hospital staff to a session on community-based solutions to manage chronic diseases and reduce unnecessary emergency department admissions and reduce 30-day readmissions.

This hourlong webinar will:

  • Highlight a library in Texas was able to successfully bridge the digital divide by helping patients access telehealth visits through access to technology and technical support. 
  • Offer an open forum in the session’s second half for sharing best practices, successes, learnings and collaboration related to telehealth.

Dianne Connery holds a Master of Library Science from Texas Woman's University and a Master of Gerontology from the University of North Texas. She is actively invested in efforts to improve broadband access and digital inclusion in rural communities. She is passionate about inspiring others and advocating for the work of rural libraries, with an emphasis on addressing the technology needs of the community.
