CKD and Diabetes: Guidelines, Best Practices and Opportunities to Improve Care

You're invited to join Comagine Health on Tuesday, Nov. 15 for a short session on the updated ADA guidelines and treatment landscape of chronic kidney disease (CKD) and the importance of screening/testing type 2 diabetic patients for kidney disease.

Participants will learn about:

  • Diagnosis codes for risk-adjusted scoring for populations with a high disease burden, and the high cost for patients
  • The current landscape of CKD/ESRD and cardiovascular risks, including racial health disparities
  • The importance of doing both tests for an accurate diagnosis: the uACR and eGFR as indicators of kidney health and early predictors of cardiovascular death
  • Drivers in CKD progression
  • Improving access to guideline-directed therapies to improve patient outcomes
  • Electronic health record and patient education best practices
  • Information on the new CKD HEDIS quality measure

Our featured speaker will be Brian Hocum, PharmD, MS, director of Bayer's Data Generation and Observational Studies.
