Change in Condition Five Week Learning Series

This five-week series will help locations decrease Emergency Department visits and hospital readmission rates by focusing on early identification of change in condition. Participants will learn how to improve clinical competencies and focus on improving workflows and outcomes for their residents.   

Join us for this interactive learning series that will help participants: 

• Identify the components of a successful change in condition program 
• Recognize changes in condition 
• Improve responses to interventions 
• Integrate change in condition work into Quality Assurance and Performance Improvement (QAPI) 
• Enhance provider communication 

Those who participate for all five weeks of the series can earn 2.5 hours of nursing continuing education hours.
