Caring for End-Stage Renal Disease Patients: A Learning Event for CHWs and Caregivers

End-stage renal disease (ESRD) is a medical condition in which a person’s kidneys stop working, leading to the need for dialysis treatment or a transplant. People living with ESRD have unique care needs that require proper caregiver training and skill-building.
Comagine Health invites community health workers (CHWs) and caregivers to join us for a learning event outlining best practices CHWs can utilize to care for people living with ESRD and prevent hospitalization.
Participants will be able to:
Describe the basics of ESRD and dialysis
Identify two dialysis modalities
List three signs of dialysis access infection and prevention strategies
Assist and support patients with medication and diet best practices
