Caring for End-Stage Renal Disease Patients: A Learning Event for CHWs and Caregivers

End-stage renal disease (ESRD) is a medical condition in which a person’s kidneys stop working, leading to the need for dialysis treatment or a transplant. People living with ESRD have unique care needs that require proper caregiver training and skill-building.

Comagine Health invites community health workers (CHWs) and caregivers to join us for a learning event outlining best practices CHWs can utilize to care for people living with ESRD and prevent hospitalization.


Learning Objectives

Participants will be able to:

  • Describe the basics of ESRD and dialysis
  • Identify two dialysis modalities
  • List three signs of dialysis access infection and prevention strategies
  • Assist and support patients with medication and diet best practices
