(HI) Electronic health exchange (HIE) allows health care providers and patients to appropriately access and securely share a patient's vital medical information electronically--improving the speed, quality, safety and cost of patient care.

Chronic conditions, such as heart disease, are a growing problem in the United States. In an effort to combat this issue, some health care organizations are implementing HIEs to more effectively manage the health of patients with chronic conditions, like cardiovascular diseases, and reduce preventable hospitalizations and adverse events.
