Comagine Health's Role — Case Management
Comagine Health's case managers are licensed professionals with extensive training and experience. Case management is free, optional support for individuals with a catastrophic injury or illness or a chronic medical condition.
Comagine Health case managers can:
- Identify health care concerns
- Gather information to help you make the best health care choices possible, within your benefits plan
- Serve as a resource for your health care team and family
- Coordinate referrals
- Assist in obtaining health care services to promote the best outcomes
