Rights and Responsibilities

As an individual receiving services through the Oregon Health Plan, you have certain rights and responsibilities under Oregon Administrative Rules (OARs). Please see OAR 410-120-1855 for all your rights and responsibilities.

In addition, Comagine Health will ensure you:

Are involved in every step of the person-centered service planning process

Person-centered means the focus is on you and guided by you. When meeting to create an individual plan for you, Comagine Health case managers ensure you guide:

  • Where you want to live
  • How you want to participate in your community
  • Your treatment goals
  • The services and support you need to meet these goals
  • Who else, if anyone, you want to be involved in your treatment and support

If someone has legal decision-making authority for you, they may choose to be a part of the process and make decisions on your behalf.

Have access to information that is explained and written in a way that you understand

  • This includes in speaking and in writing.
  • If your primary language is not English, we will give you information in your primary language.

Are treated with dignity and respect and not discriminated against by others

  • Comagine Health shall treat all individuals with dignity and respect and due consideration for privacy and choices.
  • Comagine Health shall not discriminate against individuals eligible to enroll in the program based on health status or need for healthcare services.
  • Comagine Health shall not discriminate against individuals eligible to enroll in the program based on race, color, gender, religion, national origin or sexual orientation.

Person-Centered Service Planning

Person-centered service planning can help you learn more about your personal goals. This includes how and where you want to live, and how you want to participate in your community.

Person-centered service planning also helps determine what supports and services are needed to help you move toward your goals. This information will help develop a person-centered service plan that is specific to your needs and goals.

We pledge to make sure your voice is heard and that you are an active participant in the person-centered service planning process.

What this means:

  • The person-centered service planning process is all about you.
  • You are in charge of the planning process.
  • The person-centered service planning process occurs at times and places that are convenient for you.
  • You choose who works with you to develop your person-centered service plan. You can choose who you want to assist you in making decisions.
  • You will choose what supports and services you want and need; this support may come from family, friends, staff, or someone who has legal decision-making authority in your life.
    • If you have someone with legal decision-making authority, they may choose to be a part of the process and may choose to make decisions on your behalf.
  • During this process, we will share information with you in a way that you can understand it. For example:
    • People will speak to you in the same language you speak
    • We will provide materials in your language
    • Or we will use other ways of communication that work for you
  • If a conflict or disagreement arises during the planning process, we will provide support and information throughout the process to support your needs and choices.
