The Utah Million Hearts Coalition, in conjunction with the national Centers for Disease Control Million Hearts® initiative, aims to prevent heart attacks and strokes by improving blood pressure measurement and control. The coalition is working toward this goal by encouraging and recognizing excellence in hypertension management.
The current focus is on improving the accuracy of blood pressure measurement in Utah. High blood pressure is a leading cause of heart disease and stroke in the United States, and nearly 1 out of every 2 adults (116 million) in the nation has hypertension. Measuring blood pressure accurately is the first step in improving hypertension management.
Measuring Blood Pressure Properly
Evaluate your clinic using this self-assessment, then use the resources and tools on this webpage to help your clinic achieve excellence in hypertension management. The self-assessment can be completed by anyone on the health care team — or by multiple people on the team. As you are responding to the questions, make sure you are thinking about how each staff member operates. Marking “yes” indicates an element is 100% in place — everyone on the team participates every time blood pressure is measured.
There is also an action plan template at the end of the document for any identified areas for improvement. You can work with your team to prioritize improvement and develop a plan for action.
Once you have completed the self-assessment, you can email it to us if you’d like a member of the coalition to assist you with implementing your action plan.

Honoring Primary Care Clinic Champions
Each year, the Utah Million Hearts Coalition honors primary care clinics for their efforts to effectively measure and control blood pressure for their patients. In 2022, 113 primary care clinics were recognized with the Excellence in Blood Pressure Measurement and Control Award. These clinics have met the highest standards of blood pressure measurement and control through proven evidence-based strategies, such as patient education on how lifestyle choices impact blood pressure, precise measurement of blood pressure, engaging patients with a treatment plan to improve medication adherence, fully utilizing the electronic health record for hypertension care and working as a team to ensure positive outcomes for patients with hypertension.

Your clinic can work toward becoming a Utah Million Hearts Coalition recognized champion by utilizing the resources available through the Utah Million Hearts Coalition and the national Million Hearts Initiative.
