In these modules, you will learn the definition of person-centered care and heart it described by many different experts who have followed its principles for years, and how TRACK, a self-reflection tool, can be used when providing care for older adults. This online course was developed by Making Oregon Vital for Elders (MOVE) on behalf of Oregon Department of Human Services – Aging and People with Disabilities.  Funding for this project was provided by a grant from the Oregon Quality Care Fund.
