Documents community health needs of HCWC’s quad-county region and each of the counties (Clark County, Washington, and Clackamas, Multnomah and Washington counties in Oregon). Community health needs, referred to in this report as priority health issues, were identified through a comprehensive study of population, hospital, Medicaid and community data.

Mobilizing for Action through Planning and Partnerships uses health data and community input to identify the most important community health issues. HCWC uses this modified version of the MAPP model for its assessments.
