Patient Safety
We are committed to improving the quality and safety of health care for all patients, across all settings of care. Our patient safety programs support providers, community stakeholders, patients and their families in bringing better care to the communities we serve.
Practice Transformation
Practice transformation increases efficiencies, drives down cost through value-based payments and improves patient outcomes. We put care teams and delivery networks on a proven path to improvement by guiding teams through a change management process that is both transformative and sustainable.
Community
Community engagement is a core differentiator in our work. We work with health care partners — including providers, patients and other stakeholders — to align our efforts and initiatives to improve health care in our communities.

End Stage Renal Disease
Our ESRD program creates a collaborative environment between dialysis professionals, providers and patients to improve patient care. For five states, hundreds of dialysis facilities and thousands of patients, we actively engage stakeholders to achieve ESRD-related quality measures while making sure the patient voice is included throughout our work.
