This clinical algorithm developed by the National Kidney Foundation provides an overview of appropriate screening, identifying, classifying, and managing CKD in primary care practices. It also provides helpful guidance regarding patient safety considerations based on a patient's estimated GFR, reducing cardiovascular risk, screening for complications, and when to refer to nephrology.
