In this video you will learn about long COVID prevalence, symptoms and sequela. You will hear about the impact of long COVID with respect to care access and associated barriers to care. Information is shared about how technology can increase care access and actions that can be taken at the patient level, community interpersonal level, and health care facilities and health care systems levels.

This presentation focuses on measuring success. You will learn about three types of measures, ways to assess health equity in care and identify at least one resource to support implementing a health equity lens into practice.

This session provided a deeper look at how incorporating the 4Ms of an Age-Friendly Health System into practice will positively impact the care provided to residents and reduce the potential risk of harm. Participants explored the potential impact of implicit bias on teamwork and resident care and the importance of cultural humility when interacting with others. One center shared how they successfully implemented the core elements of an age-friendly health system and how this approach improved resident-centered care. 

This session focuses on changing the narrative around change fatigue. Participants explored strategies to avoid and overcome change fatigue, assess readiness for change, and maintain continuity when leadership changes. The case study highlights the potential impact of change on residents. Strategies presented in the session will be connected to elements for improving joy in work in support of staff satisfaction and retention.

The Centers for Medicare & Medicaid Services (CMS) Quality, Safety & Education Portal (QSEP) offers an interactive, scenario-based course that incorporates recent best practices from nursing homes. With this training, front-line staff and management have an opportunity to fine-tune their skills and enhance their ability to address challenges related to COVID-19.

This session focused on recognizing and addressing the impact of change fatigue on staff, residents, and family members, to reduce the potentially negative effects on quality improvement efforts and the quality of care. Large and small group discussion opportunities allowed participants to explore strategies for building a culture of psychological safety that supports engagement to reduce the risk for change fatigue.
