Quality Incentive Program, Dialysis Facility Reports and Dialysis Facility Compare
ESRD Quality Incentive Program
The Centers for Medicare & Medicaid Services (CMS) administers the End Stage Renal Disease Quality Incentive Program (ESRD QIP) to promote high-quality services in renal dialysis facilities. The ESRD QIP links a portion of payment directly to facilities’ performance on quality of care measures.

Performance Score Reports and Certificates
Dialysis facility staff can access their Performance Score Reports (PSRs) and Performance Score Certificates (PSCs) and participate in the annual preview period via the ESRD Quality Reporting System (EQRS).
In order to submit clarification questions and formal inquiries, facilities must have a Point of Contact established through the HARP/EQRS registration process. Facilities can also apply through HARP/EQRS for facility viewer access to the CMS ESRD QIP system. Facility viewer access allows you to download and review the PSR but does not allow you to submit clarification questions and formal inquiries.

Dialysis Facility Reports and Dialysis Facility Compare
Facilities can access their Dialysis Facility Reports (DFRs) and Quarterly Dialysis Facility Compare (QDFC) Preview Reports on DialysisData.org.
Dialysis facility staff access to DialysisData.org is updated annually by each facility’s local Master Account Holder (MAH). Facilities can update their MAH with the Network by completing the Facility Personnel Update Form. The Network sends updated passwords to MAHs annually.
