Wellness Integrated Network Of Nevada

Comagine Health is working collaboratively with diverse community partners to develop a Nevada statewide infrastructure to deliver and sustain evidence-based chronic disease self-management education (CDSME) programs and interventions. The Wellness Integrated Network of Nevada (WINN) is a collaborative effort co-led by Comagine Health, Access to Healthcare Network, and Dignity Health, Quality Technical Assistance Center.

Hardly a day goes by without primary care in the news, whether it’s a new payment model, a new assessment of the primary care crisis, or the beginning or end of another business venture. The value of primary care is clear: Robust evidence from many settings shows that increased engagement with primary care improves health outcomes and patient experience, and that increased investment in and access to primary care lowers total health care costs. Based on the definition of value as quality divided by cost, primary care delivers some of the highest value in health care.

Southern Nevada Pathways Community HUB

Comagine Health is collaborating with local partners to improve health and health equity in Clark County, Nevada. By establishing the Southern Nevada Pathways Community HUB (SNPCH or the HUB) to serve as a sustainable care coordination network, we are building a safety net for community members. SNPCH uses the Pathways Community HUB Institute (PCHI) Model® and is preparing to seek certification through PCHI.
